Pre-Commercialization Fund Proposal
Commercialization Plan
The Commercialization Plan is the primary
document used for commercialization candidate review and ranking. This plan
should include the information listed below. All information will be kept
confidential if so marked.
1.
The nature of the company’s business, identifying products and services
that have been successfully commercialized in the past 5 years or are proposed
for commercialization.
2.
A copy of the company’s financial report and/or the latest Annual Report.
3.
A statement indicating if the company qualifies as a small business firm.
4.
An overview of how the company plans to use the licensed technology,
including any products that will be developed and their potential customer base.
5.
A 3-to-5 year pro forma income statement with supporting data.
6.
A chart showing project milestones with accompanying timelines and
demonstration/validation milestones.
7.
A statement of the estimated manpower, finance, and other company
resources believed required.
8.
A statement of the field(s) of use in which your company intends to apply
the technology.
9.
Define anticipated assistance required from the patent holder.
10.
A product demonstration statement or plan to be used to determine the
level of success achieved at the end of the commercialization activity.
11.
Funding desired from the Pre-Commercialization Fund.
12.
Proposed royalty rates including up-front fees and yearly minimums.
13.
Completion of the NTCI project proposal cover sheet.
14.
Completion of the licensing form as applicable.
